Critical Needs Grant

The Critical Needs Grant is funded by Preble County Board of DD levy dollars to provide supports, services and help to enable a family to care for their family member with a developmental disability at home. The supports range from home modifications to special adaptive equipment to therapy. Before completing the application, prior authorization is required to be reimbursed. Also, your SSA can assist your family to find other resources before suggesting a Critical Needs Grant.

To qualify for a Critical Needs Grant, the individual with a developmental disability must be living with the family member who is completing the application and must be eligible for DD board services. The grant request must be specifically related to the disability or diagnosis. The grant is not income-based, rather, it is a grant of last resort. If another agency cannot fully fund a request, the PCBDD may work with other agencies and the family to meet the need. Individuals receiving waiver services or foster care services are not eligible for grant funds.

Identify the support needed and call your SSA (937-456-5891) to complete the application process. The completed application is then forwarded to the Critical Needs Grant committee for review. Applicants are then notified of the committee’s decision.
